Description of Services:

Our board certified wound clinicians use both live video and wound images to evaluate a patient's wound and consult with the provider (SNF, LTAC, CAH, Home Health, Hospice) to recommend the most effective treatment for better outcomes, improved patient quality of life, and lower overall cost. Our mobile app allows a patient, a caregiver, or an onsite clinician to easily take wound photos, document the status of the wound and initiate a live video call with Corstrata's wound experts. Through our cloud-based telehealth platform, our experts monitor the wound's progress and recommend treatments that promote healing. Corstrata's HIPAA compliant technologies facilitate documentation which reduces the provider's legal liability. We help healthcare providers become Centers of Excellence for wound care by partnering with the provider for their wound care program. Our services range from comprehensive wound care management to a menu of services to enhance your current program: Wound program assessment; Staff education and annual competencies; Formulary review and assessment; Consultations for complex wounds; Pressure ulcer prevention program; Documentation review for compliance and defensibility. We help payers case manage patients with complex wounds, promoting healing and prevention of wound recurrence. We also facilitate management of the wound patient as they transition from one care setting to another- often the time when the patient is most vulnerable to inconsistent, poorly coordinated care.

Additional company information:

Corstrata Testimonials-"Due to increased financial obligation and limited availability of certified wound care nurses for ongoing evaluation and treatment recommendations, home health agencies generally do not have access to certified wound care nurses. Corstrata allows home health agencies full access to certified wound care nurses to provide these practices while retaining profit margins and giving nurses access to their vast supply of knowledge and insight within the app at a very affordable cost. Corstrata has improved our compliance with wound documentation, privacy and has brought a sense of excitement to wound care for our staff." "As a five-star skilled nursing home in Savannah, GA, Azalealand focuses on delivering the highest quality care to our residents. Partnering with Corstrata has enhanced the quality of our wound care and prevention programs. Corstrata is opening the door to rural facilities who do not have the resources locally of wound care physicians or clinics. This will enable rural facilities to deliver quality wound care without having to transport their residents to clinics far away." "It's been my pleasure to have the opportunity to work virtually with Corstrata. Their expert wound care nurse, that lives in another state, has been available by mobile app to watch over my shoulder and give guidance as I care for a non-healing plantar surface ulcer in an uncontrolled diabetic. Corstrata has made it possible to decrease the size of the wound without the patient having to see a wound care specialist. Since my patient lacks funding, wound care outside of my primary care office was a financial impossibility. This is a service that I would recommend for all providers that are concerned about providing high-value care."
