Certintell is a care management company that enables safety-net providers to make a lasting impact on the health of underserved patients through telehealth and Remote Patient Monitoring (RPM). We do this by using our in-depth expertise in health care and health information technology to anticipate — and meet — the needs of health care payers, providers and patients. We host a comprehensive suite of cellular-connected RPM devices, ready to be sent to your patients with little-to-no set up. Because of its cellular capacity, the RPM devices do not require Wifi connectivity, reducing the impact of restrictive social determinants of health, such as geographical location or financial status. Device readings are automatically transmitted to supervising clinicians. Furthermore, our triage process ensures your clinical team receives critical information when you need it, such as out-of-range alerts. Providers can connect with patients about their data on our proprietary telehealth platform, JoinCareTeam, as well as report out on accumulated readings using our proprietary health data reporting platform, ConnectCareTeam. While telehealth workflows may seem like an arduous process to implement, we make it as seamless as possible. Your team of clinicians will be backed by dedicated leads and your patients will be led toward positive behavior change by our certified clinical health coaches. The entire process is supervised by our Medical Director and Advanced Registered Nurse Practitioner. Operating as a Managed Services Organizations (MSO), Certintell ensures community-based organizations can implement sustainable telehealth programs as well, as we can currently bill Medicare and Medicaid on your behalf. As a part of our commitment to diversity, we prioritize hiring individuals who reside in the same communities as the patients we serve. Hiring individuals who may have similar backgrounds as patients offers an additional layer of expertise and increases comfortability for the patient. After all, patients trust and engage more with providers who are similar to them, rather than providers who are far removed from their lived experiences. Even though we are a predominately remote company, we find that this endeavor has been valuable for further connecting with patients and supporting their needs.

We ensure that our telehealth services are able to reach those that have Limited English Proficiency. We use an integrated language interpretation tool in our telehealth platform that can translate more than 200+ languages. Additionally, we have clinical health coaches that are bilingual, speaking both English and Spanish to meet patients where they are. This effort is just one example of our larger commitment to improving health equity. As the first black solo-founded telehealth company in the world, Certintell strives to partner with health systems to take proactive steps that address disparities that have been holding our communities down for so long.
